The North Zone PCN Business Unit has funding available to provide Practice Facilitator (PF) and Proactive Care Coordination Assistant (PCCA) services to PCNs and physician clinics that require this support but do not have PCN funding to support it.

Practice Facilitators play a key role in the Patient’s Medical Home (PMH) by assisting clinic teams with improvement towards the pillars of the PMH model. The role includes working with PCN team members, physicians, and clinic staff to advance work processes related to patient access, patient attachment, panel management, and optimization of EMR functionality.

Proactive Care Coordination Assistants (PCCAs) are administrative team members in the Patient’s Medical Home, providing outreach and proactive panel management support. PCCAs work 100% remotely with no in-clinic work. PCCAs can be shared across several physician clinics.
